Breathlessness, Not Just Coughing, Could Signal Hidden Lung Issues, Doctor Warns

While a persistent cough is often perceived as the primary sign of lung issues, it’s not the only symptom to watch out for. According to Dr. Jeenam Shah, a consultant in chest medicine at Jaslok Hospital and Research Centre in Mumbai, the absence of a cough does not necessarily equate to healthy lungs. Several lung conditions can develop without a constant cough, making it crucial to be aware of other warning signs.

Dr. Shah emphasizes that although coughing is a familiar indicator of lung problems, it is not a definitive measure of lung health. She explains that there are several respiratory conditions where symptoms may be subtle or non-existent in the early stages. Three such conditions where a persistent cough might not be present include asthma, Chronic Obstructive Pulmonary Disease (COPD), and lung cancer.

In asthma, individuals may experience symptoms such as breathlessness, wheezing, or chest tightness, often without a cough. These symptoms can occur during physical activity, at night, or after exposure to allergens or pollution. Similarly, COPD may present with increased breathlessness, making tasks like climbing stairs or walking more challenging, especially for smokers who might dismiss these changes as signs of aging.

Early stages of lung cancer may not cause any noticeable symptoms. When they do appear, they might include a persistent or changing cough, coughing up blood, chest pain, unexplained weight loss, or frequent chest infections. People at risk, particularly those with a smoking history, should discuss their risks with a healthcare provider rather than waiting for symptoms to manifest.

Other conditions, such as pulmonary hypertension and blood clots in the lungs, can also lead to unexplained breathlessness without a cough. Dr. Shah points out that breathlessness often precedes a cough and is a critical warning sign. She highlights pulmonary fibrosis as an example where changes in breathing and exercise capacity may be the first symptoms noticed, as the lung tissue becomes scarred and stiff over time.

Dr. Shah advises that changes in breathing are significant and should not be ignored. Individuals experiencing unexplained or progressive breathlessness, wheezing, a decline in exercise capacity, recurrent chest infections, chest pain, coughing up blood, or unexplained weight loss should seek medical attention. Depending on the symptoms and risk factors, a doctor may suggest tests such as spirometry, chest imaging, or oxygen assessment to diagnose potential lung issues.
